In-home Assessment and Evaluation
Our comprehensive assessment
includes a review of physical/mental
limitations, strengths, social interests,
outside contacts, sensory loss,
medical history and status, nutritional
status, safe environment, legal/financial
planning, health insurance quality
and cost control. We provide ongoing
service only after we have evaluated
your needs and have explained a
plan of service with the expected
results and cost of services. All
of our services are confidential,
and information is only disclosed
with your permission.
Care Planning
A plan of action is developed
to address immediate needs and anticipated
long term needs. We identify issues
before they become problems, and
resolve them. We can offer unbiased
suggestions for additional services
that you might need. On-going monitoring
can prevent costly crises and unnecessary
hospitalizations.
Care Coordinating and Management
We coordinate all services to
your home including transportation,
shopping, staffing, meals and more.
We can accompany our clients to
medical and other appointments and
then streamline the sometimes complicated
and confusing process involved in
managing your health care related
issues. We provide customized services.
We don’t just arrange those services;
we stay involved to be sure that
our plan of action is successfully
implemented.
Crisis Intervention
We try to be available 24 hours
a day, 7 days per week for emergency
needs of our clients and their families.
Nursing Home Placements
If our client's needs increase
more than what can be safely met
at home or is financially feasible,
we have first hand knowledge of
local nursing homes, assisted living
residences and other independent
living options. We will offer you
options that best meet your needs
and preferences. We can then assist
with the application process and
other steps that must be arranged
before, during and after admission.
We can coordinate a move to or
from home, assisted living complex,
or a nursing home, in or out of
state, including assistance with
applications, transferring medical
records, arranging for packing/moving
belongings and having the room ready
upon arrival. On-going monitoring
services are available to help make
your loved one's transition successful
